School Certification Form

Oregon Green Schools Association Certification Form

This form is to be submitted byRegional Coordinators ONLY.  Schools do not complete these forms.

Directions:
Once a school has completed its Oregon Green School Application and submitted to its Regional Coordinator for review, Regional Coordinators must submit to OGSA:

  1. The Certification Form (click HERE to complete the Certification Form – will open in a new window)
  2. The Signature Authorization Form (click HERE to download signature authorization form)
  3. A final copy of the School Application

Both the signature form and the final school application can be uploaded and submitted through the Certification Form.

Please note that this certification form must be completed each time a new school certifies, or if a currently certified school re-certifies at any level, or levels up.

Date of certification will be the month processed by the OGSA Program Coordinator, which will also be listed on the certificate.

All certifications are valid for 3 years, to the month they are certified.
