This form is for Regional Coordinators ONLY.  Schools do not fill out this section.

Please complete the form below in order for the school to be officially registered and to order its award. This must be done each time a school certifies or re-certifies at any level. Date of certification will be the date processed by the OGSA Administrative Assistant, and will be on the certificate.
